It is the spring of 1895, and more than a decade of combating eldritch entities has cost Dr. John Watson his beloved wife Mary, and has nearly broken the health of Sherlock Holmes. Yet the companions do not hesitate when they are called to the infamous Bedlam lunatic asylum, where they find an inmate speaking in R’lyehian, the language of the Old Ones. Moreover, the man is horribly scarred and has no memory of who he is. The masterful stories in this collection relate Holmes and Watson's involvement with the case of an eminent physiologist inexplicably savaged by his faithful wolfhound, of the priceless stolen Crown diamond found in the pocket of a peer, and of the mysterious figure who means to buy Mrs Maberley's house and all of its contents, whatever the cost, while Holmes himself tells the story of a Boer war hero set to inherit a fortune who mysteriously disappears from the family seat.

All six episodes of the radio series spoofing the adventures of the famous sleuth. Deep in the bowels of the British Museum, a locked leather portmanteau is unearthed. Contained within it is a rare treasure: the hitherto unpublished journals of Dr John H Watson. When the preserved foot of a dead man with extra toes arrives at St. Bartholomew's Medical College, the students are fascinated. However, despite this unusual feature being reported in the press, the man's identity remains a mystery. Arthur Conan Doyle was a GP before he became a writer. He uses his medical knowledge widely in the Sherlock Holmes stories. He bases the deductive skills of his hero detective on the diagnostic techniques a GP uses with a patient. He even gives Sherlock a GP sidekick. This all contributes to the enduring popularity of the Sherlock Holmes stories, over 130 years after the first story was published.

An author who kills off his hero in so dramatic a fashion relatively early in that hero’s literary career (and then of course returns him to life in an equally dramatic manner) has, perhaps, little else to offer us in the final stories of his famous character. Yet in this final series of short stories Conan Doyle does not cease from developing his famous detective, giving us a fascinating selection of tales.

The first British edition and the first American edition of the collection were both published in June 1927. However, they had slightly different titles. The title of the British collection was The Case-Book of Sherlock Holmes (hyphenated "Case-Book"), whereas the title of the American edition was The Case Book of Sherlock Holmes ("Case Book" as two words).
