Eines Tages tauchte ein Mr. Blessington auf, der ihm ein lukratives Angebot machte: Übernahme aller Kosten gegen einen Anteil vom Gewinn der Praxis. Trewelyan willigte ein; beide Männer lebten fortan in einem Haus und Blessington wurde sein Dauerpatient.

Der junge Arzt Percy Trevalyan erhält ein Angebot, das er nicht ablehnen kann. Ein reicher Gönner richtet ihm eine eigene Praxis ein. Gegen Beteiligung und das Recht auf lebenslange, kostenlose Behandlung. So weit, so gut. Doch plötzlich verhält sich der alte Blessington seltsam. Wähnt sich verfolgt. Verfällt in panische Zustände. Trevalyan schaltet den Meisterdetektiv ein.

Der Hausarzt Dr. Trevelyan konsultiert Holmes und Dr. Watson auf Drängen seines Dauerpatienten Mr. Blessington, bei dem bereits zweimal eingebrochen wurde, ohne dass irgendetwas fehlt. Der Fall erfährt eine unerwartete Wendung, als Mr. Blessington erhängt in seiner Wohnung aufgefunden wird.
